MetLife United Healthcare patient letter

To all our patients who have United Healthcare and MetLife Dental Insurance:

I regret to inform you that my office will be cancelling our preferred provider contract with United Healthcare Insurance Company and MetLife Dental Insurance Company effective 01 January 2021.

United Healthcare and MetLife are the only insurance carriers we must cancel at this time.

Below is a list of the Preferred Provider (PPO) insurance options we will continue to be contracted with if you wish to change your insurance plan during your upcoming insurance open enrollment.

Blue Cross/Blue Shield
Cigna
Delta Dental
Guardian
The Principal
United Concordia

We hope you will continue your dental care with us, but if you do have a previously scheduled appointment in 2021, and would like to cancel, please do so as soon as possible. As always, we will submit your insurance claims for you but will no longer accept their reduced fee. You would, therefore, need to pay at the time of service and collect payment directly from United Healthcare and MetLife.
